Autofill PDF form using Excel

I am a Visual Basic newbie and I need some help with the following code. Basically, what I am trying to do, is to autofill a PDF form using data in an excel worksheet. I did some testing and the program seems to be reading the required data from the excel cells but not writing it in the PDF form. Any ideas why??

I have attached a sample PDF and excel file. The code is in the macro.
