Automation

My problem is I have a questionnaire form that people fill out when they become members. Then I manually type their information (name, city, state, etc...) into a document that I created thats more dressed up so I can print it out and give it to them. What I want to know is how can I automate that process so when they fill out that questionnaire it automatically fills out the doument as well when they press submit or something? Manually filling out the info is a real time consuming task.
